About Topamax

What is Topamax?

Topamax (topiramate) is a sulfamate-substituted monosaccharide and an anticonvulsant medication. It is usually prescribed when other anti-seizure medications haven't been effective in patients aged 2 years or older with Lennox-Gastaut syndrome, epilepsy, or other disorders with seizures. Other Topamax uses include prevention and minimization of migraine attacks in patients over 12 years old. Occasionally it is prescribed for alcohol dependence. Topamax cannot stop migraines once they are happening. It is available as a tablet or as a capsule (Topamax Sprinkle).

How do I take Topamax?

Your dosing will be determined by your doctor, and they may choose to start you on a low dose and increase it over time. Do not take more or less than prescribed.

Normally you will take 1 pill 2 times daily at the same times every day. The Topamax pills can be taken with or without food. They should not be broken and should be swallowed whole. Topamax Sprinkle capsules should either be swallowed whole, or opened and sprinkled over a single spoonful of soft food (e.g., applesauce). Swallow the spoonful whole without chewing. What if I forget to take a dose of Topamax?

If you miss 2 or more doses of Topamax medication, call your doctor immediately. Do not take extra medication to make up for a missed dose. Unless 6 hours have elapsed since your scheduled dose, you may take your missed dose as soon as you remember it. If more than 6 hours have passed, skip the missed dose. Maintain your normal dosing schedule.

What happens if I overdose on Topamax?

Call your local poison control center or seek emergency medical assistance, especially if the person taking Topamax has collapsed or is not breathing. Symptoms of overdose include: depression, agitation, trouble thinking, trouble with speech or coordination, drowsiness, double vision, fainting, or convulsions.

Before taking Topamax, tell your doctor if:

  • You have any eye problems or vision disorders (e.g., glaucoma).

  • You have or have a history of kidney or liver disease or disorder.

  • You are on dialysis.

  • You are taking any urinary tract or bladder medications.

  • You have osteoporosis, osteomalacia, or any reason for brittle bones.

  • You have a growth disorder.

  • You have breathing problems or any lung disorders.

  • You have or have a history of mood or mental disorders, or are taking medication for such, especially lithium.

  • You have a history of suicidal ideation or actions.

  • You are taking birth control.

  • You are pregnant, plan to become pregnant, or are breastfeeding.

  • You regularly drink 2 or more alcoholic beverages daily.

  • You are taking any cold or allergy medications, whether prescribed or over-the-counter.

  • You are taking medication for IBS or stomach ulcers.

  • Your diet is currently high in fats and low in carbohydrates.

  • You are currently taking any other seizure medications or birth control pills.

  • You take medication to treat glaucoma.

  • You are taking any other prescription or non-prescription medications, drugs, vitamins, or supplements.

What are the possible side effects of taking Topamax?

Seek medical assistance or contact your doctor immediately if you experience any of these severe Topamax drug side effects: difficulty breathing or feeling short of breath, swelling of the face or tongue and throat, hives, confusion or memory problems, slowed thinking or difficulty thinking, difficulty speaking, sudden vision problems or vision loss, eye pain, fever, diminished sweating, unusual tiredness or feeling like you may faint, loss of appetite, vomiting, irregular heartbeats, severe pain in your side or lower back, painful or difficult urination.

Common Topamax drug side effects only need medical attention if they persist or get worse. These include: rapid eye movements, changes in your sense of taste, mild numbness or tingling in your arms or legs, flushing or warmth, headache, dizziness, weight loss or loss of appetite, stomach pain, nausea or diarrhea, feeling nervous, mild memory problems, mild confusion, some mood problems.

How do I store Topamax?

Always keep your Topamax pills tightly closed in the container it came in, and store at the cool end of room temperature. Keep this medication away from heat, light, and moisture (e.g., a bathroom).

Warnings

It is possible for this medication to permanently affect your vision. If you experience sudden vision changes or sudden pain around your eyes, immediately get emergency medical attention.

Topamax can sometimes cause people to become depressed or have suicidal ideations. Family members or caregivers should watch for symptoms and changes in mood.

This medication makes you more likely to dangerously dehydrate in hot weather.

You should not suddenly stop this medication without your doctor's advice, even if you become pregnant. If you do become pregnant while taking Topamax, contact your doctor immediately to discuss the pros and cons of taking this or any other seizure medication during pregnancy.
