(a) Personally Identifiable Information. To use the FamilyWize Service you may be required to provide us with certain Personally Identifiable Information as described below. We may collect some or all of this information through various forms and in various places through the FamilyWize Service and our Site, including account registration forms, contact us forms, or when you otherwise interact with us.

In addition to any Personally Identifiable Information or other information that you choose to submit to us, we and our third-party service providers may use a variety of technologies that automatically (or passively) collect certain information whenever you visit the Site or access or use the FamilyWize Service ('Non-Personally Identifiable Information'). Non-Personally Identifiable Information may include the browser that you are using, the URL that referred you to our Site, and how and when you use the FamilyWize Service. We may use Non-Personally Identifiable Information for various reasons, such as providing and enhancing the FamilyWize Service for you and other users. In addition, we may collect your IP address or other unique identifier that identifies the device from which you access the Site, FamilyWize Service. This identifier is a number that is automatically assigned to your device and which our computers use to identify you and your device This information may be non-identifying or may be associated with you. (b) Geo-Location Information. We may collect information as you navigate our Site or use the FamilyWize Service, which may include geographic location.

(d)Web Beacons. Small graphic images or other web programming code called web beacons (also known as '1x1 GIFs' or 'clear GIFs') may be included in our web and mobile pages and messages. (e) Embedded Scripts. An embedded script is programming code that is designed to collect information about your interactions with the Site and FamilyWize Service, such as the links you click on. The code is temporarily downloaded onto your device from our web server or a third party FamilyWize Service provider, is active only while you are connected to the FamilyWize Services, and is deactivated or deleted thereafter.
We may, from time to time, supplement the information we collect about you through our Site with outside records from third parties in order to provide the FamilyWize Service to you, enhance our ability to serve you, to tailor our content to you and to offer you opportunities to purchase products or FamilyWize Services that we believe may be of interest to you. We use third party companies and individuals to facilitate our FamilyWize Services, provide or perform certain aspects of the FamilyWize Services on our behalf, and other third-parties to host the FamilyWize Services, design and/or operate the FamilyWize Services' features, track the FamilyWize Services' analytics, process payments, engage in anti-fraud and security measures, provide customer support, provide geo-location information to our drivers, enable us to send you special offers, perform technical FamilyWize Services (e.g., without limitation, maintenance FamilyWize Services, database management, web analytics and improvement of the FamilyWize Services' features), or perform other administrative FamilyWize Services. We may provide these vendors with access to User Information, including Personally Identifiable Information. This information sharing is limited to only the information needed by the vendor to carry out the FamilyWize Services they are performing for you or for us. Each of these vendors are obligated not to disclose or use Personally Identifiable Information for any other purpose and when required by law, SingleCare will enter into special contracts with the vendors strictly limiting the vendors' ability to use and disclose Personally Identifiable Information and requiring the use of certain security measures by the vendors.
Certain aspects of the FamilyWize Service may be provided to you in association with third parties that will be disclosed and identifiable to you, and we may disclose Personally Identifiable Information to such parties in accordance with applicable law.
If we receive requests for information from government and law enforcement officials and private parties in regards to enforcing and complying with the law and in connection with private law suits, we will disclose User Information to these parties as we reasonably determine is necessary to comply with applicable law. For example, we may disclose User Information: (i) to satisfy any applicable law, regulation, subpoenas, governmental requests or legal process; (ii) to protect and/or defend our Terms and other policies; (iii) to protect the safety, rights, property or security of the Company, our FamilyWize Service or any third party; (iv) to protect the safety of the public for any reason; (v) to detect, prevent or otherwise address fraud, security or technical issues; and/or (vi) to prevent or stop activity we may consider to be, or to pose a risk of being, an illegal, unethical, or legally actionable activity.
We may, to the extent permitted by law, share all User Information with our parent, subsidiaries and affiliates for internal, business related (non-marketing) purposes. Behavioral Advertising uses information collected about an individual's web or mobile browsing behavior such as the pages they have visited or the searches they have made. This information is then used to determine which advertisements should be displayed to a particular individual.

Our FamilyWize Service is not directed to nor intended for minors. No one under 18 is allowed to register with or use the FamilyWize Service. As such, we do not knowingly collect Personally Identifiable Information from anyone under the age of 18. If we discover that we have collected Personally Identifiable Information from a person under 18, we will delete that information immediately.
